As you evaluate results reported in the articles, consider using the 4 rules that we discussed last week. It is very important to identify the purpose of the study before moving forward with the evaluation of the results. If the study has an intervention or treatment, then it is likely that there will be dependent and independent variables (Chamberlain College of Nursing, 2017). The next step focuses on identification of data collection methods and how investigators measured these variables. Furthermore, discussion and conclusion sections of the report offer an interpretation of study results and may even incorporate investigators’ opinions or speculations regarding the findings (Chamberlain College of Nursing, 2017).
A well-written research report contains strengths and limitations. Concerns with study limitations are especially important when investigators report results from a pilot study. Pilot studies are often designed to preliminary test the intervention to identify potential benefits; however, as we acknowledged, results from such a study are very hard to generalize to a greater population. Pay special attention to the sample selection for the study and resources needed to implement the proposed change. Last week we discovered that even though some results appear to be statistically significant, they may not have any clinical significance. The week 5 RRL article I will be discussing on is quantitative balance and gait measurement in patients

with Frontotemporal dementia and Alzheimer diseases. A pilot study. Fall is one of the leading cause of injury morbidity and mortality in older adults. People with history of falling may limit their activities because of fear of falling. Frontotemporal dementia and Alzheimer’s disease are neuro-generative dementia with a wide prevalence of fall. The purpose of the study was to identify the relationship which gait, balance, and ambulation have with falls in normal aging versus patients with AD and FTD. Identifying the patterns of gait and balance in these patients helps to put in place appropriate interventions to prevent falls in these individuals. According to the study, balance and gait problems exist in normal elderly as well as patients with AD and FTD during dual tasking indicating the role of divided attention. The study states that patients with FTD have a tendency to tilt forward, while patients with AD have a tendency to tilt backward. These indicates the different patterns of gait and balance in patients with FTD and AD. Differentiating gait and balance in these patients will help in formulating an individualized care plan for preventing falls. Working in long term care, I encounter many patients with dementia. Often times, it is very difficult to distinguish between patients with FTD and AD. There is no distinction between the types of dementia. The two main diagnoses of dementia that is common are unspecified dementia with behavioral disturbances and unspecified dementia without behavioral disturbances. Before applying the evidence in my practice, it must be a multidisciplinary approach involving every member of the healthcare team. The physician must first diagnose the patient with FTD or AD, and the interdisciplinary team will determine which intervention is appropriate for the patient by applying evidence from the study.

Evidence based research is very important in nursing. Research and evidence based practice go hand in hand with each other. Research leads to new knowledge and nurses use research findings as evidence during their practice. I will share the research based evidence with my peers by conducting meetings and in-services. Publishing the results of the research findings in the organization’s bulletin board, newsletters, and websites. I can also use statistical analysis such as tables, graphs, and pie charts when presenting research based evidence. Our week 7 lesson states that the research process is terminated by disseminating or communicating information about the research study through publications. An example of a clinical issue is the implementation of an evidence based clinical practice guideline to reduce the number of center acquired pressure ulcers on wheelchair and bed bound patients. Using a quantitative question in the research also provide a stronger level of evidence when sharing research based evidence with my peers. Quantitative research seeks to make conclusions about the effectiveness of an intervention. They can be broken down into PICO elements (population, intervention, comparison, and outcome) for easy understanding (Houser, 2017).

The article I chose from the Week 5 RRL assignment is “Making Sense of a New Technology in Clinical Practice: A Qualitative Study of Patient and Physician Perspectives”. The purpose of this study was to determine how new technology that detects Cardiovascular Autonomic Neuropathy (CAN) was perceived by the physicians and the patients of a specialist diabetes clinic. When new practices are implemented, it is important to explore the physician’s understanding and the patient’s understanding of the practice or technology. The physicians had difficulty explaining the complexity of the Cardiovascular Autonomic Neuropathy test and its results on a level that patients would understand. Because the physicians were not able to explain the information, the patients were not influenced to modify their physical behavior or diet. According to the authors, the study suggests that open-communication and respect during communication with the patients would more likely yield positive results.
The findings from this study are reference a Cardiovascular Autonomic Neuropathy test but could be applied to the use of any new equipment. Per Houser, for the research to be trustworthy, it must have credibility, confirmability, dependability, and transferability (Houser, 2018, p. 393). I believe this evidence is trustworthy and could be applied in my practice. As a member of the medical team, we are constantly obtaining new equipment and learning how to use it. Recently, a sales representative brought a new high flow oxygen system into our Emergency Department for the medical team to trial. The purpose of the system is to decrease the need for bi-pap. For a patient that has never seen this machine, education must be given prior to using it or the patient may feel anxious. If the provider does not have a good understanding of how the machine works, he cannot explain the process to the patient.
My clinical issue from week three was compassion fatigue in the Emergency Department. I would review the statistical data that I obtained through the study. I would make a PowerPoint presentation and email it to the department. I would then follow-up with each person individually to ensure that he / she understands the information contained in the PowerPoint. If the statistical data shows that education decreases compassion fatigue amongst staff, I will educate the rest of the department. I will also make sure that all new-hires from this point forward are given the education during new employee unit-based orientation. The most significant barrier that I can foresee is that people often resist changes.

The research article titled “Using Quantitative Balance and Gait Measurement in Patients with Front temporal Dementia and Alzheimer Disease: A Pilot Study” (Velayutham et al, 2017) describes a research study conducted concerning older patients with Alzheimer’s. The goal was to gather more evidence to help prevent falls from seniors. In this study, the researchers discover that balance and gait issues persist in seniors with Alzheimer’s and frontemporal dementia, and also with patients without dementia when attempting to perform dual tasks. In this study the research showed that patients who suffer from frontemporal dementia are more likely to fall forward while those with Alzheimer’s are more likely to tilt backwards. The conclusion is that patients with FTD and AD may have different biomarkers than those who are aging normally. I would apply this evidence to my practice, with some considerations. The research here is sound, but the sample size is just too small to make a generalization of the scope needed to influence practice. However, this is an important and helpful study that helps protect vulnerable senior patients. I would request this study be replicated very often across different settings with larger sample sizes and random assignment of groups. The basic purpose of predicting fall direction and preventing falls in seniors is extremely significant.

Some information found in my nursing clinical issue of preventing hospital acquired pressure ulcers in patients is that the cost of treating pressure ulcers is very high. It is estimated that the average cost for treating a pressure ulcer is $50,000 (Soban, 2011). This includes factors like: supplies for would care, staff time dedicated to wound care, specialty beds, and nutritional support (Soban, 2011).
Methods for sharing information would include the most basic method of word-of-mouth. Using the Internet to disseminate information is also another way to share research based evidence. Sharing information to nurse managers and directors of nursing is a way to reach influential people who can affect needed change and incorporate pressure monitoring technology into everyday practice. Many people are also visually and inclined, so engaging in visual learning with charts, graphs something like a PowerPoint presentation to peers would also be another method of share research based evidence. For financial officers, emphasizing the link between hospital acquired pressure ulcers and financial burdens placed upon the hospital is another effective way of sharing evidence.

The Quantitative Balance and Gait Measurement study had a ghastly amount of charts, statistics and measurements that took up literally half of the study. (3/6 printed pages minus the reference page) The data was hard to understand and the authors used histograms, columns, tables and some crazy octagon pie chart. I personally could care less for all of the P(ANOVA), and all of the statistical information that was overloaded in this particular study. I found it humorous in the end when I read the first sentence in the conclusion,” This study reveals balance and gait problems in normal elderly, as well as patients with AD and FTD…”(Velayutham. et al.2017). The reports could of used just 1 chart format and cut the statistical information down considerably.
This study made me think of something funny I read, There are too many scientific studies, says scientific studies (Matyszczyk, C., March,2017).
