Healthcare delivery methods continue to evolve as care providers respond to complex patient needs. Central to this evolution is mHealth, which allows patients to manage their health using various mobile apps. Mental health, eating behaviors, physical activity, and other health aspects can be managed via health apps. As a result, nurses should help patients understand the use of different apps according to their diverse health conditions and needs. Patient education also helps patients actively participate in disease management to achieve the desired health outcomes. The purpose of this paper is to develop a patient education plan on the use of a mHealth app for diabetes management.
Patient Scenario
Health problems vary across patients depending on how they experience healthcare disparities. Education, income, and social class determine where and how people live, affecting their health immensely. Approximately 37 million people in the United States have diabetes, underlining the need for improved management in healthcare facilities and homes (Centers for Disease Control and Prevention, 2022). Poorly controlled diabetes increases the risk of kidney damage and vision loss, among other health complications. Healthy People 2030 primary focus is reducing diabetes cases, related complications, and deaths and improving the quality of life of patients and at-risk individuals (U.S. Department of Health and Human Services, n.d.). Achieving such outcomes is a foundation of better health for individuals and at-risk populations.
Frank Jones presents himself at the community health center with type 2 diabetes, whose management has become a significant family problem. Frank has struggled to keep glucose levels within the desired health range in the past six months. Other concerns include carb intake control and timely data sharing with his doctor. As a result, he wants advice and medical assistance on overcoming these problems and achieving good health. Based on his concerns and questions, Frank shows readiness to learn about the effective management of type 2 diabetes. The focus of patient education should be showing him how to use mHealth for better disease management.

Your initial discussion post should be a minimum of 200 words and response posts should be a minimum of 150 words. Be advised that I grade based on quality and not necessarily the number of words you post. A minimum of TWO references should be used for your initial post. For your response post, you do not need references as personal experiences would count as response posts.
If you however cite anything from the literature for your response post, it is required that you cite your reference. You should include a minimum of THREE references for papers in this course. Please note that references should be no more than 5 years old except recommended as a resource for the class. Furthermore, for each discussion board question, you need ONE initial substantive response and TWO substantive responses to either your classmates or your instructor for a total of THREE responses.
There are TWO discussion questions each week, hence, you need a total minimum of SIX discussion posts for each week. I usually post a discussion question each week. You could also respond to these as it would count towards your required SIX discussion posts for the week.
I understand this is a lot of information to cover in 5 weeks, however, the Bible says in Philippians 4:13 that we can do all things through Christ that strengthens us. Even in times like this, we are encouraged by God’s word that we have the ability in us to succeed with His strength.
