Thank you for doing such a great job with the definition and examples for the four spheres of influence or political action in nursing.  All of these sphere while they each have a different role have to play a part to complete the whole in the nursing profession.  We might not think we are using each one during our nursing practice until we really look back and think about it.  Something you said in your post about not having much involvement in the community influence of nursing I think is incorrect.  By being able to teach and be involved with new nurses, you are helping to form the kind of nurse they want to be, which becomes reflective of the care that patients receive at the hospital spreading this to the community around the hospital.  Also when nurses have a good and positive starting point in their nursing career it helps to show them the many sides of nursing and what they might be capable in other settings beside inpatient.  I have experience starting as an inpatient nurse and moving to community practice within the same hospital system, but my job is so different and the patient interactions I have influence patients to make changes in their lives.  I feel that my time as an inpatient nurse has given me a lot of knowledge and confidence that has helped many of the community patients I work with as well.
I would agree that ethics is such a large part of our profession, hopefully most people’s profession.  I think as nurses though we tend to face more ethical dilemmas more often than most people in their work lives.  Something that I think people either forget or really do not know is: “that Nursing has its own body of scientific and technical knowledge, built and reproduced by a set of social, ethical and political practices carried out through teaching, research and care.(Nunes da Silva, Moreira Freire, Ferreira de Vasconcelos, Vital da Silva Junior, de Carvalho Silva, da Silva Araujo, & Assis Eloy, 2018, p.4) As nurses we have to remember this as well and understand impact we make with patients can lead to impacting our workplace, organizations and even government.  We have the ability to make changes on a bigger scale, and should work to pursue that more.  Daily as nurses we are making changes in the lives of our patients by showing them kindness, compassion, empathy, and sharing knowledge.  These skill can translate into making more global changes.  I think what you said about the nursing ratio is really interesting and how people have left where you work due to the fact that the census has changed.  I think that there needs to be more regulations on ratios for nurses, because there is a lot expected of us, but when you make us take more patients mistakes can happen and do more often.  Also we do not get the time to spend with a patient as what nursing is recognized for as caregivers.  I am not sure if you have a staffing grid at your hospital, but we do and obviously depending on census we will have less nurses with our census down, so we get use to having about the same number a patients on every shift we work.  I think having that consistency helps nurses have perspective on their current place of employment.  I think a lot of the time we think the grass is greener on the other side, but when we get there it usually is worse, maybe not related to staffing ratios, but other things that we did not think about.  Ethically I think it is important to have safe ratios everywhere and I believe that would help with the turn- over of staffing more.

The four spheres of political actions in nursing are the workplace, government, community and professional organizations (Mason et al, 2012).   Each one of the spheres has it’s own separate functions such as: the workplace focuses on issue, which affect jobs and patient care. Government addresses rules, laws, and manages regulations in nursing practice. Community involves issues that affect community well-being and lastly organizations which address concerns related to shaping nursing practice (Mason et al, 2012). Together the spheres can create change. Nurses are able to change policy making and improve issues in the community health systems (Mason et al, 2012).
When you start out in your new nursing career, it is vital to review the political policies and nursing policies in your organizations. Nurses don’t realize that we are already political our nursing practice is molded around governing bodies, ethical, professional, governmental standards, and healthcare itself (Bjornsdottir, 2009).
I believe in centered family care in our organization. Especially when a trauma has occurred and the outcome doesn’t look good. It is important for family to see that you have done everything you can to help their loved one. Nursing has the greatest power in this situation (Parker, 2013). We are responsible for being the patient’s advocate. In the workplace it is vital for the patient’s family to be at their child’s bedside when something critical is taking place. If not then the family is wondering what is being done on their child (Parker, 2013) when a family member becomes involved they know you have done your best even if the outcome is not good. We have an ethical responsibility to do everything we can to save someone’s life. Nurses have to remind surgeons and other physicians that families have a right to be with their loved ones (Parker, 2013). Physicians see families as a distraction and that they will be in the way, instead of seeing them as a positive and realizing the family needs to believe that we tried everything to save their child’s life. If not allowed in the room they decide what they think or don’t think you did for their loved one (Parker, 2013).
Some ethical problems one may face with not allowing patient centered care in your organization can be wait times in the emergency room. These extended wait times puts a patient and their families at risk for harm (CDC, 2014). The main reason for this is boarding of patient’s for lack of nurses to take care of patient’s inpatient. When boarded patient’s take up Ed resources which makes waiting times in the Ed waiting room increase to unsafe levels (CDC, 2014).
