Addressing Small Problems to Build Safer, More Reliable Systems
Health care delivery requires progressive developments to improve essential health outcomes, including care quality, patient safety, and patient satisfaction. Direct practice improvement (DPI) projects and other practice change programs are instrumental in reducing gaps between the achieved and desired health outcomes. Their successful implementation and sustainability are the foundation of safer, more reliable systems.
From a health dimension, my DPI project achieves clinical improvement since the knowledge derived from it and its implementation will significantly improve health care quality. In the advancing nursing practice, care quality should be a priority for nurse practitioners, leaders, and stakeholders since its improvement leads to better health outcomes and experiences (Backhouse & Ogunlayi, 2020). Adverse outcomes such as health complications and mortality are reduced through projects that produce knowledge for care quality improvement.
Nurse practitioners must think from a broader perspective as they engage in projects that achieve clinical improvement. The implication is that nurse practitioners should think of how DPI projects will achieve widespread change. The most effective strategy for achieving widespread change through the current project is results dissemination. In quality improvement projects and other outcome-oriented initiatives, dissemination in practice change entails sharing research findings with others to maximize their use in improving health outcomes (Ashcraft et al., 2020; Ross-Hellauer et al., 2020).

A similar approach will help to share knowledge derived from the current project with other practitioners within and outside the study setting. The widespread change would be slower if its implementation were in a country with universal health care. Besides investing in sustainable systems, countries with universal health care ensure that people get quality health care services when, where, and how they need them (Zieff et al., 2020). Despite this provision, quality improvement is universal. Hence, the project would still be relevant in such countries.

The health outcome would be impacted positively. Irrespective of the implementation area and approach, progressive changes to improve clinical outcomes help to address practice gaps. Although the process might be slower in some countries due to resource challenges, patients and health care practitioners eventually benefit from programs that strive to improve clinical outcomes. However, a culture of change and managerial support are crucial for effective project implementation.

The Commonwealth Fund provides an international review of health care systems. Read the Commonwealth Fund report, International Profiles of Health Care Systems, 2014, and take the associated quiz. Discuss what knowledge you gained from the quiz and describe any changes you would apply to the U.S. health care system.
Hello Stefanie, it is incredible to see the depth of your understanding regarding the social determinants of health (SDOH) and how they disproportionately affect vulnerable populations, particularly Black LGBTQ+ single parents in rural areas. Your insights into the challenges faced by Black women are compelling.
Racial discrimination, economic instability, and limited access to quality healthcare create a web of adversity. The intersectionality of being Black, LGBTQ+, and a single parent magnifies these challenges (Medina-Martinez et al., 2021). The disparities in wages, health outcomes, and preventive screenings underscore the urgent need for targeted interventions.
Your proposal of universal healthcare, incentives for healthcare providers in rural areas, and improved education on gender/sex identity aligns with a comprehensive approach to address these systemic issues. Universal healthcare would break down financial barriers, while incentivizing providers to work in rural areas could bridge the healthcare access gap (Sutton et al., 2021). Education on gender/sex identity is crucial for fostering inclusivity and understanding, ultimately contributing to improved healthcare experiences.
Your acknowledgment of the complex challenges, including government policies, structural issues in healthcare, and discrimination, demonstrates a holistic understanding. The emphasis on active listening and fostering collaborative relationships as a future healthcare provider is commendable. Patient-cantered care is pivotal in addressing the unique needs of individuals within these vulnerable populations.
Integrating risk assessment tools like root cause analysis and the DMAIC Model reflects a strategic and systematic approach to problem-solving. Identifying core issues and measuring the effectiveness of interventions through DMAIC aligns with evidence-based practices, ensuring that efforts lead to meaningful improvements (Ponsiglione et al., 2021).
As you continue your journey in advocating for health equity, your thoughtful analysis and proposed changes provide a strong foundation for positive impact. Your commitment to understanding and addressing the multifaceted challenges faced by vulnerable populations is truly commendable.
