Systems based thinking requires nurses and other health care professionals to see “the big picture” in relation to patient-care, and improving health outcomes through ensuring continuity of care for patients in all health care settings. This requires establishing relationships, effective communication skills, and collaboration amongst all interdisciplinary team members involved in patient care. Public health nurses must use a systems-based approach in order to work with their communities to promote health, and prevent disease and disability. Reducing health disparities requires nurses to advocate for equitable health care access by eliminating socioeconomic and geographic barriers (“Health,” 2020).
As part of the initiative to address health disparities in the King County region(s), public health leaders established a goal to provide patient-centered care through increased access to primary care and social services, as well as improve care coordination through integration of physical and behavioral health care services (“King,” 2020). All too often, patients struggling with mental illness only seek care during an emergent or acute crisis. This ineffective and fragmented delivery of care is costly, and uses up unnecessary resources in an already understaffed and overworked health care system (“At-risk,” 2020). A community health center specific to my region is called International Community Health Center (ICHS). It provides care for thousands of patients each year in 11 different locations throughout King County with the goal to provide culturally competent health care services to improve wellness in King County’s diverse communities (“International,” 2017). In addition to providing on-site medical and dental services, the Shoreline medical and dental clinic also provides access to nutrition programs (such as WIC), an on-site pharmacy, and a young adult clinic, catering to 14-26-year olds and provides low-cost/free access to behavioral and reproductive health services. This helps improve continuity of care through access to multiple health care services all in the same primary care setting.

In society, health inequalities are affected by an individual’s socioeconomic situation, level of education and environment. These health inequalities also affect a group’s health disparity. In order to change a groups health disparity and inequality a systemic approach is needed just like an interdisciplinary approach is needed for an individualized care. My community is heavily populated with hispanics and would most benefit from a systems-based approach.“The Centers for Disease Control and Prevention has cited some of the leading causes of illness and death among Hispanics, include heart disease, cancer, unintentional injuries (accidents), stroke, and diabetes”(U.S Department of Health and human services office of minority health. para. 6).
In this case, in order to address the issue of diabetes in hispanics a systemic approach is needed. On the individual level patient teaching is important. The disease process, signs and symptoms, complications of the disease process and the ways to manage this chronic illness needs to be taught. The health care team needs to stress the importance of preventative care such early screening. An individualized assessment needs to be conducted with each health care visit. Nurses must coordinate with the individual on making their medication available in accordance with their preferred pharmacy. Social workers need to be involved in providing an individual with available resources and coordinate with them ways to maintain ongoing care. This can include providing visiting nurse services, or arranging transportation via access -a -ride. The government needs to provide individuals with accessibility to affordable care. Due to the fact that many hispanics live in a poor economic area many do not have access to health insurance or can’t afford to pay the premiums. The government needs to provide legislation on lowering the cost of insurance by adjusting their annual budget. Another thing the government needs to do is insure health care provided in clinics and hospitals are all up to standards everywhere despite the geographic location. By using the systemic approach the individual, the care team, the organization and the government work together to address the health issues of the community. There needs to be constant feedback and interaction among these parts in order to have an effective health care system.

You must proofread your paper. But do not strictly rely on your computer’s spell-checker and grammar-checker; failure to do so indicates a lack of effort on your part and you can expect your grade to suffer accordingly. Papers with numerous misspelled words and grammatical mistakes will be penalized. Read over your paper – in silence and then aloud – before handing it in and make corrections as necessary. Often it is advantageous to have a friend proofread your paper for obvious errors. Handwritten corrections are preferable to uncorrected mistakes.
Use a standard 10 to 12 point (10 to 12 characters per inch) typeface. Smaller or compressed type and papers with small margins or single-spacing are hard to read. It is better to let your essay run over the recommended number of pages than to try to compress it into fewer pages.
Likewise, large type, large margins, large indentations, triple-spacing, increased leading (space between lines), increased kerning (space between letters), and any other such attempts at “padding” to increase the length of a paper are unacceptable, wasteful of trees, and will not fool your professor.
The paper must be neatly formatted, double-spaced with a one-inch margin on the top, bottom, and sides of each page. When submitting hard copy, be sure to use white paper and print out using dark ink. If it is hard to read your essay, it will also be hard to follow your argument.

Discussion Questions (DQ)
Initial responses to the DQ should address all components of the questions asked, include a minimum of one scholarly source, and be at least 250 words.
Successful responses are substantive (i.e., add something new to the discussion, engage others in the discussion, well-developed idea) and include at least one scholarly source.
One or two sentence responses, simple statements of agreement or “good post,” and responses that are off-topic will not count as substantive. Substantive responses should be at least 150 words.
I encourage you to incorporate the readings from the week (as applicable) into your responses.
Weekly Participation
Your initial responses to the mandatory DQ do not count toward participation and are graded separately.
In addition to the DQ responses, you must post at least one reply to peers (or me) on three separate days, for a total of three replies.
Participation posts do not require a scholarly source/citation (unless you cite someone else’s work).
Part of your weekly participation includes viewing the weekly announcement and attesting to watching it in the comments. These announcements are made to ensure you understand everything that is due during the week.
APA Format and Writing Quality
Familiarize yourself with APA format and practice using it correctly. It is used for most writing assignments for your degree. Visit the Writing Center in the Student Success Center, under the Resources tab in LoudCloud for APA paper templates, citation examples, tips, etc. Points will be deducted for poor use of APA format or absence of APA format (if required).
Cite all sources of information! When in doubt, cite the source. Paraphrasing also requires a citation.
I highly recommend using the APA Publication Manual, 6th edition.
Use of Direct Quotes
I discourage overutilization of direct quotes in DQs and assignments at the Masters’ level and deduct points accordingly.
As Masters’ level students, it is important that you be able to critically analyze and interpret information from journal articles and other resources. Simply restating someone else’s words does not demonstrate an understanding of the content or critical analysis of the content.
It is best to paraphrase content and cite your source.
