Walden NURS-6001 Week 4 Discussion Using the Walden Library
I am currently pursuing the family nurse practitioner program at Walden University. An area that interests me is endocrinology in the primary care setting. I feel that many patients are choosing to have their primary care team oversee their diabetes management as opposed to going to a specialist. It can become very stressful for patients to have to see many different doctors and in certain cases, it is completely necessary to see a specialist. Still, I do believe there are certain diseases that primary care should be able to treat such as Type 2 diabetes adequately. I began my search on Walden’s Library page. Walden’s Library page offers many different resources and databases.
I started first by using CINAHL & Medline combined search, “CINAHL stands for Cumulative Index to Nursing & Allied Health Literature, and includes peer-reviewed articles, reports of evidence-based practice, case studies, systemic reviews, and more.” (Walden, n.d). Upon entering this site, I used two terms, endocrinology and primary care. I came across a peer-reviewed article called An endocrinologist-supported intervention aimed at providers improve diabetes management in primary care site: improving primary care of African Americans with diabetes which concluded by explaining, “Feedback on performance aimed at overcoming clinical inertia and giving to internal medicine resident primary care providers improves glycemic control. Partnering generalists with diabetes specialists may be important to enhance diabetes management in other primary care settings.” (Phillips, et al, 2005). As a focus in my studies, I want to overcome clinical inertia and put ADA standards into practice, and feel comfortable with adjusting PO and insulin regimens for optimal glycemic control in the primary care setting.
Difficulties I experienced while searching for an article were upon typing in my search over 4,000 articles come up, I don’t think it is possible or realistic to be able to shift through every single article. It took me some time to choose an article that spoke to what I was researching. Many of the articles were abstract instead of being able to access full text.
I believe this database would be extremely useful for my colleagues as we are all in these graduate studies programs that require access to scholarly articles and evidence-based practice to apply that research in our practice. The online article Simplifying Synthesis states, “Quality evidence-based practice initiatives and nursing care are based on understanding and evaluating the resources and research available, identifying gaps, and building a strong foundation for future work.” (Shellenbarger, 2016). Understanding and evaluating resources and research available is vital for professional growth so I would definitely recommend this database as I felt that it provides access to quality evidence-based practice while being easy to navigate.

Your discussion post was encouraging. I appreciate your ideas that emphasize health equity and accessibility in underserved communities. To further support your thoughts, Dahlin et al. (2022) composed an in-depth case study of Mr. Jones to promote culturally congruent care for the diverse population. Dahlin et al. (2022) suggested that nurses bear a significant role in social change to recognize differences in culture, values, and practices, evaluate social determinants of health that influence response to illness and treatment, and provide potential resources to improve their health and quality of life. Evidence-based research is imperative for relevant topics, like health equity and accessibility, in today’s society because professionals can construct innovative care techniques based on credible evidence, clinicians’ knowledge and skills, and patients’ preferences.
I also utilized the CINAHL PLUS database throughout my undergraduate studies and respected the amount of information available to scholars. With the recommendation to the CINAHL PLUS database, I want to add that all scholars can narrow their search to a specific timeframe (Walden University, n.d.). For example, relevant evidence-based research is usually based on a 5-year timeframe, so any scholar can select only to view articles from 2018-2023. The CINAHL PLUS database will be an excellent resource to support our thoughts and ideas toward innovative, evidence-based research.

Advanced Psychiatric and Mental Health Nursing Practice
The area of practice that is interesting to me is the advanced practice in psychiatric and mental health nursing. This area of nursing is very specialized and a huge need across the nation. Upon beginning this research, it is a very essential skill as an advanced practice nurse to be able to identify and appraise the best available evidence-based research. It is vital as a nurse to be able to assess the credibility of articles that are researched to be able to incorporate them within your own clinical experiences and practices (Al-Jundi & Sakka, 2017). The research engine that I utilized in the Walden University Library is EBSCO. EBSCO has been recognized as one of the top research databases, e-journals, magazines, e-books, and subscription services to libraries of all kinds. They are aligned with other libraries to help improve the quality of their content and technology (About EBSCO: EBSCO, n.d.). I think this database would be very useful for both students and future colleagues. This database has developed a large search engine to provide the most information possible for evidence-based research.
According to this article I found, it has been noted that the research specific for psychiatric and mental health nurse practitioners (PMHNP) is limited due to it being a subgroup of the broader advanced practice registered nurses (APRNs) (Phoenix & Chapman, 2020). Until recently, the amount of PMHNP programs have grown over the last 10 years. This is a significant rise; behavioral health systems are now hiring more PMHNPs due to the need for providers to treat patients and prescribe their medications (Phoenix & Chapman, 2020). Due to the need in this field and my passion to care for mental health patients is why I chose this field to practice.
